Clinical stage T4 breast cancer represents an aggressive subset of breast cancer associated with poorer survival outcomes compared with earlier-stage breast cancer. While it has been shown that breast reconstruction improves long-term patient satisfaction and quality of life in early breast cancer,
1 the impact of breast reconstruction and timing of reconstruction on satisfaction and quality of life after mastectomy is not well understood for cT4 patients. For patients with clinical stage T4d (inflammatory breast carcinoma [IBC]) breast cancer, immediate reconstruction has been associated with a higher risk of complications and delays in radiotherapy. The standard of care for IBC is to avoid immediate reconstruction and remove all involved breast skin.
2 Even for non-IBC clinical stage T4a-c patients, most patients elect to postpone or forego reconstruction to decrease risk for complications and potential delays in adjuvant therapy.
