Over the past decade, the key importance of the perioperative hemodynamic management for the success of microsurgery has been acknowledged. Adequate regulation of systemic blood pressure is fundamental to ensure sufficient flap perfusion.
1 Concern for vasopressor associated flap complications due to the induction of vasospasm, however, has influenced intraoperative blood pressure management strategies for patients undergoing free flap surgery. The widely applied vasopressor restriction has led to a traditional overzealous fluid resuscitation.
2 On the other hand, evidence for beneficial effects of fluid restriction is growing in other surgical specialties. The main goal of our study was to evaluate the clinical effects of intraoperative fluid and vasopressor administration on tissue perfusion in free Deep-Inferior-Epigastric-Perforator (DIEP) flaps to elaborate clinical practice guidelines for intraoperative circulatory support in microsurgery.
