Bullöses Pemphigoid

Ein neuer Blick auf eine bekannte Krankheit

Hintergrund

Bei dem bullösen Pemphigoid handelt es sich um eine Autoimmunerkrankung, die mit einer subepidermalen Blasenbildung einhergeht. Im Serum der Patienten werden typischerweise IgG-Antikörper gegen Bestandteile der epithelialen Basalmembran (BP 180 und 230 Antigene) gefunden. In der direkten Immunfluoreszenz zeigt sich in der Regel eine lineare Ablagerung von IgG und/oder C3 entlang der Basalmembran, wobei auch die anderen Immunglobuline vorhanden sein können.

Fallpräsentation

Bei unserem Patient zeigte sich klinisch nicht das für ein bullöses Pemphigoid typische Bild von prallen Blasen am Körper. Das klinische Bild war eher vereinbar mit einer Prurigo simplex subacuta, bzw. einer pruriginösen Variante atopischer Dematitis. Zudem zeigte sich im Patientenserum eine starke Erhöhung des Gesamt-IgE-Wertes. Die Diagnosesicherung war durch die ELISA-Untersuchung des Serums sowie eine direkte Immunfluoreszenz von periläsionärer Haut möglich.

Schlussfolgerung

Atypische klinische Präsentationen des bullösen Pemphigoids ohne Blasenbildung sind möglich. Aktuelle Studien zeigen, dass eine Erhöhung des Gesamt-IgE-Wertes im Patientenserum häufig bei der Erkrankung nachweisbar ist. Dieser Beitrag bietet dem Leser einen kurzen Exkurs über die neuen Erkenntnisse in der Pathogenese und Therapie des bullösen Pemphigoids.
