It is with great interest that we read “Diagnostic Modality of
Clostridioides difficile Infection Predicts Treatment Response and Outcomes in Inflammatory Bowel Disease” by Gupta and colleagues [
1]. Gupta demonstrated that Toxin (+) PCR (+)
Clostridioides difficile (C. difficile) patients show robust response to antibiotics, suggesting infection as the primary driver of symptoms. Toxin (−) PCR (+) patients alternatively demonstrate response to IBD therapy escalation, thereby leading the authors to conclude that Toxin (−) PCR (+) represents
C. difficile colonization with symptoms attributable to an IBD flare. …
