Zusammenfassung

Zusätzlich zu den allgemeinen Grundregeln der Chirurgie sind in der dentoalveolären Chirurgie einige Besonderheiten zu berücksichtigen. Neben Anamnese und „klassischer“ Befunderhebung (Inspektion, Palpation) und der Standardröntgendiagnostik (Zahnfilm, OPT, FRS, Schädel p.a.) stehen bildgebende Verfahren (CT, MRT, DVT) zur Verfügung, die mit speziellen Programmen detailgenaue Darstellungen, 3D-Animationen, virtuelle Operationsplanungen und navigiertes Operieren ermöglichen. So faszinierend diese Techniken auch sind, sollte dasjenige bildgebende Verfahren gewählt werden, welches bei der geringstmöglichen Dosis eine ausreichende Information zur Diagnostik liefert (ALARA-Prinzip: „as low as reasonably achievable“). Bei der Operationsplanung erfordert die enge Nachbarschaft von oralen Hart- und Weichgeweben detaillierte Kenntnisse der Anatomie und Physiologie sowie häufig ein spezielles Instrumentarium, um den unterschiedlichen Texturen der Gewebe entsprechend Rechnung zu tragen. Häufig haben wir es mit kombinierten Knochen- und Weichteilwunden zu tun, die neben aseptischem und atraumatischem Vorgehen auch spezielle Anforderungen an Schnittführung und Nahttechniken stellen.

Zur Behandlung schwerer Formen der chronischen Rhinosinusitis mit Nasenpolypen (CRSwNP) stehen seit Kurzem verschiedene Behandlungsmethoden zur Verfügung, darunter Biologika, wie Dupilumab, und die endoskopische Sinuschirurgie (ESS). Beim Vergleich der beiden Therapieoptionen war Dupilumab leicht im Vorteil.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
