19. Evidenzbasierte medikamentöse Therapie der chronischen Pankreatitis

Zusammenfassung

Die medikamentöse Therapie der chronischen Pankreatitis (CP) ist im Allgemeinen eine symptomatische Therapie. Drei wesentliche Symptome stehen im Zentrum der Behandlung: Schmerzen sowie der Ausfall der exokrinen und endokrinen Funktionen des Organs. Etablierte kausale medikamentöse Therapien existieren nicht. Das Kapitel gibt einen Überblick über die verfügbaren symptomatischen Therapiemaßnahmen und schließt mit einem Ausblick auf vielversprechende Therapiekonzepte, für deren Anwendung bisher jedoch keine ausreichende Evidenz vorliegt. Im Speziellen wird auf etablierte und obsolete Aspekte der Schmerztherapie eingegangen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
