Dual kidney transplantation (DKT) is currently a well-established procedure for increasing the availability of organs with the use of sub-optimal grafts (Expanded Criteria Donors—ECDs). There are various protocols for deciding the allocation of kidneys for single (SKT) or dual transplantation, based on pre-implantation clinical and histological criteria. In Italy such organs are currently allocated using the scoring system introduced by Remuzzi et al. based on the Karpinski histological score. Following a general trend based on the preliminary results of studies on the short-term function of grafts, and regarding the value of pre-implant renal biopsy [

2], we have, in the last few years, extended the classic Remuzzi criteria to use kidneys with a score of 4 or 5 for single transplantation [
