19. Geburtshilfliche Zervixsonografie und Diagnostik der drohenden Frühgeburt

Zusammenfassung

Das drängendste klinische Problem der Geburtshilfe ist die Frühgeburtlichkeit. Zur Erkennung einer Frühgeburtsgefahr hat sich der transvaginale Zervixultraschall im Laufe der letzten 30 Jahre als Standardmethode etabliert. Sowohl in der Screeningsituation mit und ohne anamnestische Risiken als auch bei der notfallmäßigen Vorstellung symptomatischer Schwangerer vor Erreichen der fetalen Reife im Kreißsaal ist die Zervixsonografie ein unverzichtbares Werkzeug, zumal inzwischen wirksame und effiziente Therapieoptionen bezüglich ihrer korrekten Indikation und Durchführung genauer charakterisiert werden konnten. Hierbei kommt v.a. der Progesteron-Prophylaxe eine entscheidende Rolle zu. Die Vaginalsonographie kann somit einen wertvollen Beitrag zur Vermeidung von Frühgeburten leisten und so nicht nur menschliches Leid(en) verhindern, sondern auch im erheblichen Maß Kosten sparen helfen.

Müssen sich Schwangere einer Krebstherapie unterziehen, rufen Immuncheckpointinhibitoren offenbar nicht mehr unerwünschte Wirkungen hervor als andere Mittel gegen Krebs.

Bislang gibt es kein Medikament zur Prävention von Wochenbettdepressionen. Das Injektionsanästhetikum Esketamin könnte womöglich diese Lücke füllen.
