Ein Update zu atopischem Ekzem und Psoriasis

Zusammenfassung

Das atopische Ekzem und die Psoriasis vulgaris zählen zu den häufigsten chronisch-entzündlichen Dermatosen. Beide sind typische multifaktorielle Erkrankungen, bei denen genetische Prädisposition und Umwelteinflüsse in Entstehung und Verlauf zusammenwirken. Epidemiologische Studien, die eine Heritabilität von bis zu 80% für beide Erkrankungen beschreiben, weisen auf die bedeutende Rolle genetischer Suszeptibilitätsfaktoren hin. Anders als bei schweren monogenen Erkrankungen liegt allerdings nicht eine einzelne Veränderung in einem Gen vor, sondern ein komplexes Zusammenspiel verschiedener Varianten in mehreren Genen, deren Wirkung sich „summiert“. Auch wenn für beide Erkrankungen nach wie vor nur ein kleiner Teil der involvierten Gene bekannt ist, konnte in den vergangenen Jahren insbesondere durch die Möglichkeit hypothesenfreier genomweiter Assoziationsstudien ein erheblicher Wissenszuwachs erzielt werden.
