Heparininduzierte Thrombopenie in der Kardiochirurgie

Zusammenfassung

Im Rahmen der Immunerkrankung heparininduzierte Thrombopenie (HIT) bildet der mit Heparin antikoagulierte Patient IgG-Antikörper gegen an den Plättchenfaktor 4 gebundenes Heparin. Die formierten Antigen-Antikörper-Komplexe aktivieren Thrombozyten und lösen mitunter eine massive Thrombingenerierung aus, mit Thrombosen und Embolien als möglichen Folgen. Bei positiver Diagnose der akuten HIT, die auf klinischen Scoring-Systemen und spezifischen Labortests basiert, wird die Antikoagulation auf den direkten Thrombinhemmer Argatroban umgestellt. Während kardiochirurgischer Operationen stellt dies ein Problem dar. Sichere Alternativen zu Heparin/Protamin sind bislang nicht etabliert. Zudem sind die Empfehlungen der Fachgesellschaften hinsichtlich des Antikoagulationsregimes inhomogen. Der vorliegende Beitrag bietet eine Übersicht zu Pathophysiologie und Diagnostik der HIT. Managementoptionen während herzchirurgischer Eingriffe werden im Licht aktueller internationaler Empfehlungen diskutiert.
