Zusammenfassung

Die Therapie mit einem implantierbaren Kardioverter-Defibrillator (ICD) stellt eine prognostische und keine symptomatische Therapie dar und sollte daher auf Patienten beschränkt bleiben, bei denen eine Prognoseverbesserung möglich und sinnvoll erscheint. Eine ICD-Implantation sollte nur bei einer Lebenserwartung >1 Jahr bei adäquater Lebensqualität erfolgen. Die Entscheidung, bei welchem Patienten eine Prognoseverbesserung kein sinnvolles Therapieziel mehr darstellt, ist medizinisch und ethisch problematisch. Inwieweit ein ICD bei älteren Patienten mit konkurrierenden lebensbedrohlichen Erkrankungen die Prognose verbessert, ist ebenso unklar, wie die Definition von „alt“. Primärprophylaktisch scheint der ICD beim Älteren die Prognose in geringerem Masse zu verbessern als sekundärprophylaktisch. Alte ICD-Patienten haben jedoch eine ähnliche Rate adäquater ICD-Therapien wie jüngere. ICD-Komplikationen bei Implantation und im Langzeit-Verlauf sind beim Älteren nicht häufiger und stellen daher keinen Grund dar, die ICD-Indikation zurückhaltender zu stellen oder ein Alterslimit festzusetzen. Die Indikation zum ICD ist beim Älteren vielmehr individuell zu stellen, abhängig von verbleibender Lebenserwartung, Komorbidität, biologischem Alter und persönlicher Einstellung des Patienten, die in besonderer Weise in die Entscheidung einbezogen werden sollte, ebenso wie eine mögliche Verbesserung der Lebensqualität bei ICD-Implantation auch als antibradykardes oder Resynchronisationssystem. Die Deaktivierung zumindest der Schocks sollte diskutiert werden, wenn der Patient das Thema anspricht oder bei Verschlechterung des Zustands eine Reanimation nicht mehr sinnvoll erscheint. Sie sollte nicht erst erfolgen, wenn der Tod unmittelbar bevorsteht oder bereits ein elektrischer Sturm zu Schockabgaben geführt hat.
