Hirnstrukturelle Veränderungen bei bipolaren affektiven Störungen

Zusammenfassung

Die hirnstrukturellen Grundlagen bipolarer affektiver Störungen sind weitgehend ungeklärt. Bekannt ist aber die Neuroanatomie stimmungsregulierender Regelkreise. So konnten in einigen Regionen dieser Regelkreise strukturelle Veränderungen nachgewiesen werden. Dies betrifft vor allem eine Vergrößerung der Amygdalaregion und widersprüchliche Hinweise auf Verkleinerungen umschriebener präfrontaler Hirnregionen. Darüber hinaus gibt es erste Hinweise auf Volumenverminderungen des Kleinhirns und der Hypophyse. Außerdem scheinen Läsionen der weißen Substanz von Bedeutung zu sein. Diese strukturellen Abnormalitäten unterscheiden sich von den Befunden bei unipolaren Depressionen und der Schizophrenie. Zur weitergehenden Charakterisierung der neurobiologischen Grundlagen bipolarer affektiver Störungen sind Untersuchungen der Gehirnstruktur in Verbindung mit funktionellen Untersuchungen, wie der Magnetresonanzspektroskopie, und die Untersuchung neuroendokriner Regelkreise erforderlich. Eine parallele Genotypisierung sollte eine Subtypisierung in diagnostisch, prognostisch und therapeutisch unterscheidbare Gruppen zum Ziel haben.
