Klinisches Staging der Peritonealkarzinose durch Scoringsysteme

Eine kritische Analyse

Zusammenfassung

Im Rahmen des klinischen Stagings zur Beschreibung des Umfangs einer Peritonealkarzinose, der bei zytoreduktiver Chirurgie notwendigen Maßnahmen und des erzielten Ergebnisses in Form der Vollständigkeit der chirurgischen Zytoreduktion stehen unterschiedliche Klassifikations- und Scoringsysteme zur Verfügung. Die höchste klinische Relevanz bei der Peritonealkarzinose gastrointestinaler Tumoren haben Klassifikationen zur Tumorausdehnung und zur Vollständigkeit der chirurgischen Tumorresektion. Zumeist erfolgt eine Zuordnung der Patienten in die entsprechenden Stadien aufgrund der intraoperativen Evaluation und ist daher subjektiv. Die unterschiedlichen Klassifikationen werden dargestellt und hinsichtlich ihrer Anwendbarkeit, Genauigkeit und ihrer Wertigkeit als Selektionsinstrument sowie ihrer Aussagefähigkeit zur Prognose diskutiert und verglichen.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
