Diamond-Forrester und Kardio-CT

Muss die KHK-Vortestwahrscheinlichkeit neu definiert werden?

Zusammenfassung

Neben der vor allem in den USA verbreitet verwendeten Diamond-Forrester-Klassifikation zur Abschätzung der Prätestwahrscheinlichkeit für das Vorliegen einer signifikanten koronaren Herzkrankheit (KHK) existieren weitere Scores wie eine aktualisierte Version nach Genders et al., der Morise-Score sowie der Duke Clinical Risk Score. Anhand der Symptomcharakteristika, des Alters, des Geschlechts und ggf. weiterer Merkmale wird die Wahrscheinlichkeit für das Vorliegen einer KHK abgeschätzt. All diesen Scores gemeinsam ist ihre Evaluation an Kollektiven von Patienten, bei denen eine invasive Koronarangiographie als Referenzverfahren vorlag. Daraus resultierend zeigte sich, dass mit diesen Scores die Prätestwahrscheinlichkeit für das Vorliegen einer signifikanten KHK zum Teil erheblich überschätzt wird. Wendet man diese Risikoscores für das Vorhersagen einer signifikanten KHK in Kollektiven an, bei denen anstatt einer invasiven Koronarangiographie nicht-invasive Diagnostik wie z. B. eine Computertomographie (CT)-Koronarangiographie geplant ist, führt dies zu einer Überschätzung der zu erwartenden Prävalenz signifikanter Koronarstenosen, wodurch wiederum die Aussagekraft der Diagnostik (positiver und negativer prädiktiver Wert) beeinflusst wird. Dies muss bei der Beurteilung von Untersuchungsergebnissen berücksichtigt werden.
