4. Advance Care Planning – Vorausschauende Versorgungsplanung

Zusammenfassung

In den vergangenen 10 Jahren hat „advance care planning“ in der palliativmedizinischen Literatur und Praxis zunehmendes Gewicht erhalten. Es zeigte sich, dass eine umfassende Vorbereitung auf das Lebensende nicht nur die Verfassung von „advance directives“ erleichtert, sondern darüber hinaus die Lebensqualität des Patienten und seiner Familie verbessert. Zum einen dient die vorausschauende Versorgungsplanung der Neuorientierung und Entwicklung von individuellen Lebens- und Therapiezielen angesichts einer lebensbedrohlichen oder lebenslimitierenden Erkrankung. Die psychologischen Konstrukte der allgemeinen Lebensqualität, Sinnfindung und Hoffnung können zum Verständnis dieses Prozesses beitragen. Ziel der vorausschauenden Versorgungsplanung ist es, dass lebensbedrohlich erkrankte Kinder auch in Akut- und Notfallsituationen entsprechend den wohlüberlegten Präferenzen des Kindes bzw. seiner Sorgeberechtigten behandelt werden. Hierzu haben sich Vorausverfügungen einwilligungsfähiger Jugendlicher und „Empfehlungen zum Vorgehen in Notfallsituationen“ (EVN) für Minderjährige, nicht einwilligungsfähige Kinder bewährt. Vor dem Hintergrund der vorhandenen Erfahrungen werden in diesem Kapitel Eckpunkte und Empfehlungen zur vorausschauenden Versorgungsplanung in der pädiatrischen Palliativversorgung vorgestellt.
