Differenzialdiagnose chronisch-entzündlicher Erkrankungen des Zentralnervensystems

Liquordiagnostik und immunologische Parameter

Zusammenfassung

Eine Vielzahl neurologischer Syndrome und Erkrankungen können durch eine Beteiligung des Nervensystems im Rahmen eines systemischen Lupus erythematodes, einer Sarkoidose, eines M. Behçet sowie des Sjögren-Syndrom beobachtet werden. Dazu gehören Kopfschmerzsyndrome, Schlaganfälle, Epilepsiesyndrome, Demenzen, Polyneuropathiesyndrome und Krankheitsbilder, die der Multiplen Sklerose ähneln. Die Diagnostik ist bei z. T. fehlenden spezifischen Befunden in der Zusatzdiagnostik schwierig. Gemeinsame Befunde in der Liquordiagnostik wie eine leichte Pleozytose und oligoklonale Banden erschweren die Differenzierung zusätzlich—besonders in Abgrenzung zu entzündlichen Erkrankungen, die auf das Nervensystem beschränkt bleiben wie die Multiple Sklerose. Dennoch ist die Differenzierung eine zwingende Voraussetzung für eine adäquate Therapie und eine entsprechende Verbesserung der Langzeitprognose. Klinisch richtungsweisende Befunde sowie typische Liquorbefunde, die eine Differenzierung auch gegenüber der Multiplen Sklerose erlauben können, sollen im Folgenden dargestellt werden und ihr Stellenwert kritisch diskutiert werden.
