Medikamentöse Glaukomtherapie

Welche Lehren können wir aus den großen klinischen Studien ziehen?

Zusammenfassung

Die großen prospektiven multizentrischen randomisierten kontrollierten klinischen Studien (RCTs) Ocular Hypertension Glaucoma Treatment Study (OHTS), Early Manifest Glaucoma Trial (EMGT), Advanced Glaucoma Intervention Study (AGIS), Collaborative Initial Glaucoma Treatment Study (CITGS) und Collaborative Normal Tension Glaucoma Study (CNGTS) werden häufig als Meilensteine des Glaukommanagements bezeichnet, da ihre Ergebnisse die Evidenz für zahlreiche therapeutische Entscheidungen in der Praxis liefern. Die Studien bestätigen den bereits bestehenden Konsens, dass die prozentuale Senkung des Augeninnendrucks das Risiko der Glaukomprogression über das gesamte Glaukomspektrum von der okulären Hypertension bis zum fortgeschrittenen Glaukom reduziert. Darüber hinaus kann durch die Identifizierung neuer Risikofaktoren das Risiko der Entwicklung eines Gesichtsfeldausfalls oder seiner Progression mit größerer Präzision bestimmt werden. Die großen Studien haben unser Verständnis der Glaukome und ihrer Behandlung weiterentwickelt und eine Brücke zwischen konsensusbasierten und evidenzbasierten Entscheidungen geschlagen hat. Für die Übertragung ihrer Ergebnisse in den klinischen Alltag ist jedoch eine adäquate und akkurate Interpretation unabdingbar.
