21. Mikrochirurgische Transplantate

Zusammenfassung

Freie Lappen finden seit 1977 Einsatz in der rekonstruktiven Mund-, Kiefer- und Gesichtschirurgie und sind heute ein fester Bestandteil des Fachgebiets. Im Wesentlichen spielt dabei die Wiederherstellung nach ablativer Tumorchirurgie eine große Rolle, für die sich diese mikrochirurgisch anastomosierten Transplantate durchgesetzt haben. Bei routinemäßigem Einsatz von mikrochirurgischen Transplantaten ist eine Erfolgsrate mit kompletter Einheilung des Lappens zwischen 90–95 % zu erwarten. Dies kann sogar regelmäßig auf vorbestrahlte Patienten zutreffen. Ein mikrochirurgisches Transplantat ist für den klinischen Routineeinsatz dann geeignet, wenn 3 wichtige Voraussetzungen erfüllt sind: 1) An der Entnahmestelle des Transplantats, in der Transplantatspenderregion, sollten möglichst definierte und konstante anatomische Gegebenheiten vorliegen; 2) die das Transplantat versorgenden Blutgefäße sollten kaliberstark sowie unversehrt sein und einen möglichst variantenarmen Verlauf aufweisen; 3) die Transplantatentnahme sollte nicht oder zu nur geringfügigen funktionellen und/oder ästhetischen Beeinträchtigungen führen.

Nach PCI besteht ein erhöhtes Blutungsrisiko, wenn die Behandelten eine verminderte linksventrikuläre Ejektionsfraktion aufweisen. Das Risiko ist umso höher, je stärker die Pumpfunktion eingeschränkt ist.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
