Flap-Techniken – heute noch „State of the Art“?

Zusammenfassung

Das perianale Fistelleiden begleitet den Menschen seit jeher. Bis vor ungefähr 100 Jahren konnten perianale Fisteln nur durch Spaltung, verbunden mit entsprechend hohen Inkontinenzraten, therapiert werden. Anfang des 20. Jahrhunderts wurde die Technik des rektalen Verschiebelappens erstbeschrieben. Seither erfolgten einige Anpassungen der Operationstechnik, das Grundprinzip blieb jedoch das gleiche. In den letzten Jahren etablierten sich zunehmend neuere Techniken, die in Zukunft das gängige Verfahren der Verschiebelappenplastik verdrängen könnten. Aktuelle Metaanalysen zeigen jedoch weiterhin den hohen Stellenwert der Verschiebelappen auf Grund hoher Erfolgs- sowie niedriger Komplikations‑, Rezidiv- und Inkontinenzraten. Für die Zukunft könnte das endoskopische Verfahren der OTS-Clips zunehmend an Bedeutung gewinnen. Aktuelle Studienergebnisse können diese Fragestellung jedoch noch nicht abschließend beantworten.
