Multidirektionale Schulterinstabilität

Zusammenfassung

„Multidirektionale Schulterinstabilität“ (MDI) bezeichnet eine symptomatische Instabilität des Schultergelenks in mindestens zwei Richtungen, wobei oft eine vermehrte globale Laxität vorliegt. Zugrunde liegen meist ein durch repetitive Belastung (z. B. bei Überkopfsport) erworbenes oder kongenital vermehrtes Kapselvolumen sowie nach neueren Erkenntnissen Dysbalancen und pathologische Bewegungs- bzw. Rekrutierungsmuster der stabilisierenden Muskelgruppen. Eine traumatische MDI ist selten, obwohl vergleichbare intraartikuläre Läsionen vorliegen können. Konservative Verfahren zur Normalisierung der muskulären Störungen einschließlich Verbesserung der Rumpfkontrolle stehen therapeutisch im Vordergrund. Bei Versagen der konservativen Behandlung können heute arthroskopische Kapselreduktionsoperationen gleich gute Ergebnisse erreichen wie die klassischen offenen Verfahren bei reduzierter Invasivität und Morbidität.
