Zusammenfassung

Die Ponseti-Methode hat sich in den letzten 10 Jahren beinahe weltweit als Goldstandard zur initialen Klumpfußtherapie durchgesetzt. Dennoch gibt es nach wie vor große Schwankungen in Bezug auf Authentizität und Qualität in der Anwendung der Ponseti-Methode. Besonders die Bemühungen bezüglich Schienencompliance und folglich die Rezidivhäufigkeit zeigen eine große Streubreite. Dementsprechend sehen wir nach wie vor viele Klumpfußrezidive oder auch Residuen im Kindesalter. In den letzten Jahren konnte in großen Zentren gezeigt werden, dass selbst diese in den meisten Fällen durch eine wiederholte Gipstherapie und kleine Zusatzeingriffe wie Tibialis-anterior-Sehnen-Transfers und Achillessehnenverlängerungen gut korrigierbar sind. Nur sehr selten müssen heute schwere Rezidive bei voroperierten Klumpfüßen oder sekundären Klumpfüßen invasiveren operativen Therapien zugeführt werden.
