Patellofemorale Instabilität am kindlichen Knie

Zusammenfassung

Die akute Patellaerstluxation und die rezidivierende Patellainstabilität sind häufig im Kindes- und Jugendalter zu beobachten. Vor allem bei der Erstluxation muss auf das Vorliegen einer möglichen osteochondralen Fraktur geachtet werden. Auch bei Kindern gilt es, die bekannten Risikofaktoren der Patellaluxation zu bestimmen. Die Erstluxation ohne Begleitverletzungen (z. B. Flake-Fraktur) kann konservativ therapiert werden. Liegt ein ungünstiges Risikoprofil vor, kann primär eine operative Therapie in Betracht gezogen werden. Bei Kindern mit rezidivierender patellofemoraler Instabilität gibt es auch schon vor dem Wachstumsfugenschluss verschiedene operative Möglichkeiten der medialen weichteiligen Stabilisierung, des distalen Realignments und der knöchernen Modifikation durch Wachstumslenkung.
