Zusammenfassung

In Deutschland leiden ca. 3–6 % der Erwachsenen an klinisch relevanten schlafbezogenen Atmungsstörungen (SBAS), welche nur bei einer Minderheit der Betroffenen therapiert werden. Dies gilt vor allem für Hochrisikogruppen mit > 40 % SBAS-Patienten (z. B. adipöse Diabetiker). Die klinische Relevanz der SBAS ergibt sich entweder aus erfolgreich therapierbaren mannigfaltigen Symptomen oder aus fachübergreifend etablierten negativen Auswirkungen auf andere Organsysteme. Die häufigsten SBAS, die obstruktive und zentrale Schlafapnoe, können mit verschiedenen Formen von positivem Atemwegsdruck (PAP) effektiv behandelt werden. Zu den Herausforderungen der pneumologischen Schlafmedizin gehören die Umsetzung von bestehenden Leitlinien (z. B. arterielle Hypertonie) und die zeitnahe, qualitativ hochwertige, pneumologisch-schlafmedizinische Versorgung von vielen betroffenen Patienten. Zukünftige Forschung muss fundiert Strategien für eine individualisierte Therapie von SBAS entwickeln (z. B. vereinfachte vs. erweiterte schlafmedizinische Diagnostik), um sicherzustellen, dass jeder SABS-Phänotyp mit der für ihn medizinisch erforderlichen und ökonomisch sinnvollen Methode diagnostiziert und behandelt wird.
