We read with huge interest the manuscript of Georgios Psychogios et al. entitled “Review of surgical techniques and guide for decision making in the treatment of benign parotid tumors” [
1]. The authors suggest that extracapsular dissection provides the more effective outcome in patients with benign parotid tail tumors and can be applied as the first-line surgical technique in most patients who have benign parotid tail tumors. We agree with the authors of the publication, but we would like to pay attention to several aspects concerning the treatment of parotid tumors, especially with regard to pleomorphic adenoma. Rare (near 1%) but so characteristic of pleomorphic adenoma satellite tumor, lack of continuity, the tumor capsule and the presence of protrusions in the parenchyma of the parotid gland may be the reasons for the not radical treatment. That is why the residual disease may onset even after many years. Because residual pleomorphic adenoma may be relatively inconspicuous and various carcinoma subtypes are encountered, misdiagnosis is common. …
