We thank Dimosthenis and colleagues
1 and Liakakos and Roukos
2 for their interest in our manuscript and their thoughtful comments. We will combine their questions, because they all addressed the central issue of more aggressive regional nodal control of breast and gastric cancers, in addition to increased local control, in relation to overall survival and the question of when and how cancer cells disseminate. All authors of the letters are from the same institution and, thus, in our opinion, represent the same critique. It is of the utmost importance to distinguish between local recurrence (in the bed of primary cancer) and regional nodal recurrence (recurrence in the adjacent lymph nodes), as these are entirely different biological processes, although usually, in the literature, are inappropriately combined. …
