The false-negative rate (FNR) for melanoma sentinel lymph node (SLN) surgery ranges between 5.6 and 21%.

2 Preoperative lymphoscintigraphy with single-photon emission computed tomography and computed tomography (SPECT/CT) imaging, intraoperative localization of SLNs, and pathologic examination of these lymph nodes are important steps for the optimal evaluation of SLNs. Missing SLNs intraoperatively is a major cause for an increasing FNR in melanoma SLN surgery. The goal of this study was to show whether Sentinella (Sentinella S102, Oncovision S.A., Valencia, Spain), an intraoperative portable gamma camera (PGC), can increase identification of SLNs found by gamma probe (GP) used in the conventional surgical approach. Furthermore, we wanted to know whether any additional SLNs identified by the PGC might contain micrometastasis.
