Zusammenfassung

Die transanale endoskopische Mikrochirurgie (TEM) wurde 1983 als minimalinvasive Methode zur Resektion von Adenomen und frühen Rektumkarzinomen eingeführt, die sich für eine lokale oder koloskopische Exzision nicht eignen und sonst einen größeren Eingriff erfordern würden. Nach 25 Jahren wird immer noch über die Methode debattiert. Dieser Artikel stellt die Technik, Indikationen, Ergebnisse und Komplikationen der TEM vor, wobei der Schwerpunkt auf deren Rolle bei Rektumkarzinomen liegt. Die hier angesprochenen strittigen Punkte beinhalten die Langzeitergebnisse, TEM bei High-Risk-T1-Läsionen, TEM in Verbindung mit Kombinationstherapie [“combined modality therapy“ (CMT)] bei invasivem Rektumkarzinom und Sanierungsoperation („salvage surgery“) nach TEM. Die Zukunftsperspektiven für die TEM sind vielversprechend, und in Verbindung mit der CMT wird sich wahrscheinlich die Gruppe ausgewählter Patienten erweitern, die von der Methode profitieren.
