Zusammenfassung

Ovarialzysten sind im klinischen Alltag häufig. Die Fülle von Differenzialdiagnosen ist eine Herausforderung. Aufgrund der Unterschiede in Therapie und Prognose ist eine frühe und richtige Diagnose unerlässlich, um unnötige Operationen und Übertherapie zu vermeiden und eine optimale Behandlung durchführen zu können. Wichtigste diagnostische Kriterien sind die Anamnese, gynäkologische Untersuchung und Sonographie. Die Inzidenz der einzelnen Erkrankungen ist stark vom Menopausenstatus abhängig. Postmenopausal steigt die Wahrscheinlichkeit eines Malignoms. Wichtig ist hier, Malignitätskriterien in der Anamnese, der Untersuchung und der Sonographie zu erkennen und diese Patientinnen weiteren Untersuchungen und Therapien zuzuleiten. Prämenopausal sind funktionelle Zysten und Endometriome die häufigsten Entitäten. Asymptomatische funktionelle Zysten können zunächst sonographisch kontrolliert werden. Bei zunehmenden Schmerzsymptomen, Persistenz/Progress der Zyste oder sonographischen Veränderungen ist eine operative Klärung anzustreben. Die Therapie der Endometriome ist stark von Symptomatik und Kinderwunsch abhängig. Wichtigste Methode ist die laparoskopische Zystenbalgexstirpation.
