A female infant was delivered by emergency caesarean section at 37-week gestation because of membranous rupture and the breech position. She was the second child of a 34-year-old mother who was diagnosed with gestational diabetes mellitus. The mother had a history of imperforated anus operated at birth. She had been diagnosed with chronic kidney disease for three years before pregnancy. The mother’s estimated glomerular filtration rate (eGFR) was 49 mL/min/1.73m2 with serum magnesium 1.7 mg/dL. Echography showed normal-sized kidneys without hyperechogenicity, mild left-sided hydronephrosis, and ureteropelvic junction stenosis, which were confirmed by computed tomography. Urinalysis was normal including microalbumin, β2 microglobulin (β2MG), and N-acetyl-β-D-glucosaminidase (NAG). Serologic tests for glomerulonephritis were negative, and liver enzymes were normal. …
