Zusammenfassung

Kontinuierliche Nierenersatzverfahren (CRRT) sind die dominierende Behandlungsform des akuten Nierenversagens (AKI) bei intensivmedizinischen Patienten. Die Wahl der Antikoagulation (AK) hat dabei Einfluss auf Sicherheit und Therapieeffektivität. Relevante Blutungskomplikationen unter systemischer AK sind nicht selten und beeinflussen das Behandlungsergebnis entscheidend. Die regionale Zitratantikoagulation (RCA) stellt dabei eine Alternative mit reduzierten Blutungskomplikationen, äquivalenter Behandlungseffektivität, längerer Filterlebenszeit und geringerem Transfusionsbedarf dar. Unter adäquatem Therapiemanagement und Monitoring ist die RCA auch bei Risikopatienten ein sicheres Therapieverfahren. Die Beurteilung der Kosteneffizienz zwischen RCA und Heparin bleibt Gegenstand zukünftiger Untersuchungen.
