Antikörpertherapie bei malignem Lymphom

Zusammenfassung

Monoklonale Antikörper haben die Therapie maligner Lymphome revolutioniert. Durch den Einsatz des CD20-Antikörpers Rituximab konnten bei vielen Krankheitsentitäten Ansprechraten, progressionsfreies Überleben und Gesamtüberleben wesentlich verbessert werden. So kommt Rituximab in der Induktions-, Erhaltungs- und Rezidivtherapie des follikulären Lymphoms, Mantelzelllymphoms, diffus-großzelligen Lymphoms und der chronischen lymphatischen Leukämie (CLL) zum Einsatz. Als Weiterentwicklungen sind Ofatumumab und Obinutuzumab als vielversprechende Substanzen in unterschiedlichen Stadien der klinischen Erprobung. Weitere Antikörper haben CD22, CD80 und CD52 als Target-Molekül. Der CD52-Antikörper Alemtuzumab ist bei Patienten mit fludarabinrefraktärer CLL zugelassen und ist bei Patienten mit Hochrisikozytogenetik wirksam. Neben konventionellen Antikörpern befindet sich mit Blinatumomab ein „bispezifischer T-cell engager“ (BiTE) in klinischer Erprobung. Dieser Artikel vermittelt einen Überblick über die verschiedenen in der Lymphomtherapie zum Einsatz kommenden Antikörper.

Hodgkin Lymphom: BrECADD-Regime übertrifft die Erwartungen

Das Kombinationsregime BrECADD mit Brentuximab vedotin ermöglichte in der Studie HD21 beim fortgeschrittenen klassischen Hodgkin-Lymphom eine unerwartet hohe progressionsfreie Überlebensrate von 94,3% nach vier Jahren. Gleichzeitig war das Regime besser tolerabel als der bisherige Standard eBEACOPP.

Antikörper-Drug-Konjugat verdoppelt PFS bei Multiplem Myelom

Zwei Phase-3-Studien deuten auf erhebliche Vorteile des Antikörper-Wirkstoff-Konjugats Belantamab-Mafodotin bei vorbehandelten Personen mit Multiplem Myelom: Im Vergleich mit einer Standard-Tripeltherapie wurde das progressionsfreie Überleben teilweise mehr als verdoppelt.

Neuer TKI gegen CML: Höhere Wirksamkeit, seltener Nebenwirkungen

Der Tyrosinkinasehemmer (TKI) Asciminib ist älteren Vertretern dieser Gruppe bei CML offenbar überlegen: Personen mit frisch diagnostizierter CML entwickelten damit in einer Phase-3-Studie häufiger eine gut molekulare Response, aber seltener ernste Nebenwirkungen.
