Behandlungsoptionen beim karpalen Kollaps nach Skaphoidfraktur

Zusammenfassung

Unbehandelte Skaphoidpseudarthrosen enden in der Regel im karpalem Kollaps („scaphoid nonunion advanced collapse“, SNAC wrist). Während im Stadium I eine Rekonstruktion des Skaphoids in Verbindung mit einer Resektion des Processus styloideus radii noch erfolgversprechend ist, stehen für fortgeschrittene Stadien Rettungsoperationen zur Verfügung, um Schmerzen zu reduzieren und die Handgelenkfunktion zu erhalten. Denervation, „proximal row carpectomy“ und Teilarthrodesen erhalten die Beweglichkeit. Die vollständige Handgelenkarthrodese und der alloplastische Ersatz sind Rückzugmöglichkeiten bei Panarthrose oder fehlgeschlagenen bewegungserhaltenden Eingriffen.
