11. Die passiven Bausteine der Regulativen Schmerzbehandlung – Schmerzlinderung, myofasziale Gewebehomöostase und -funktionen

Zusammenfassung

Die Periostdruckmassage aktiviert effektiv die Schmerzhemmung, eine Komponente, die auch andere schmerzhafte Interventionen haben. Massagen wirken zusätzlich über die reaktive Hyperämie schmerzlindernd. Die Gelenksensoren reagieren vorrangig im ROM-Endbereich und bahnen u. a. die Schmerzhemmung, sodass Mobilisations- und Dehntechniken anti-nozizeptiv wirken. Massagewirkungen sind unvollständig verstanden, nur kurz anhaltend und nicht nachhaltig. Die Technik, die Intensität und die Gewebemasse sind bestimmend. Die wichtigste Wirkung ist die Durchblutung. Dehnen steigert ohne überlegenen Modus den ROM. Der konsistente Wirkfaktor ist die Schmerztoleranz. Es wirkt antifibrotisch und antientzündlich. Spinale Mobilisationen verursachen eine Hypoalgesie und aktivieren nicht konsistent kurz den Sympathikus. Die vielen passiven Techniken, Lokalisationen und Dosierungen lassen zz. keine klare Aussage zu und begründen die modifizierten Reaktionen. Die Schmerzhemmung wird gefördert. Dass erlernte Konzept und die „klinischen Erfahrungen“ bestimmen die Anwendung.
