Differenzialdiagnose schlafgebundener Ereignisse und Interaktionen

Zusammenfassung

Epileptologie und Schlafmedizin haben viele Berührungspunkte, z. B. die Differenzialdiagnose von Ereignissen im Schlaf. Stereotype simple Handlungsschablonen, v. a. in der 2. Nachthälfte, sprechen für epileptische Anfälle; Auftreten in der 1. Nachthälfte und komplexe variable Verhaltensweisen für Non-Rapid-Eye-Movement(NREM)-Parasomnien. Bei REM-Schlaf-Verhaltensstörung kommt es zu kurzen Ausbrüchen wilder, häufig gezielter Handlungen, bei denen oft aggressive Träume ausgelebt werden. Zur Diagnosestellung kann ein nächtliches Elektroenzephalogramm (EEG) inklusive Polysomnographie (PSG) erforderlich sein. Häufige nächtliche Anfälle stören den Schlaf, ein gestörter Schlaf kann andererseits das Auftreten Arousal-induzierter Anfälle begünstigen, z. B. bei Schlafapnoesyndrom oder bei periodischen Extremitätenbewegungen im Schlaf (PLMS). Viele antikonvulsive Medikamente haben Einfluss auf den Schlaf und auf die Vigilanz am Tag. Auch antikonvulsive Stimulationsverfahren können in Schlafstruktur und -qualität eingreifen.
