Fine needle aspiration biopsy (FNAB) is a safe, accurate, and cost effective tool for the initial management of thyroid nodules [1‐4]. The mean reported accuracy of thyroid FNAB is over 76 %, with a range from 53 to 95 % [5, 6]. A variety of factors can lead to diagnostic errors in the evaluation of thyroid FNABs. One feature that can be a diagnostic challenge is the presence of cells with clear cytoplasm. Such clear cell features can be seen in benign thyroid lesions as well as in more clinically significant entities that include metastasis from a primarily clear cell malignancy such as clear cell renal cell carcinoma. …
