Left ventricular non-compaction (LVNC) is a rare form of cardiomyopathy characterized by LV trabeculations seen on echocardiography or cardiac MR but its appearance on myocardial perfusion imaging (MPI) is not well established.

3 A few studies have shown abnormal MPI in LVNC, including reversible ischemia without obstructive coronary artery disease (CAD) but there have been no reports of LVNC with normal perfusion.

3 We present herein a patient with proven LVNC by echocardiography who also had MYBPC3 mutation but with normal MPI. This finding may simply represent a variant of LVNC or a lesser degree of fibrosis with negligible impact on myocardial cell perfusion and viability. …
