Herausforderungen bei der chirurgischen Therapie der Colitis ulcerosa

Zusammenfassung

Die Standardtherapie bei Colitis ulcerosa (CU) mit therapierefraktärem Verlauf oder hochgradiger Dysplasie ist die restaurative Proktokolektomie im ein- oder mehrzeitigen Verfahren unter Ileostomaschutz. Herausforderungen in der chirurgischen Therapie sind neben der Zeitpunktfrage das Resektionsausmaß und die Pouchkonstruktion inklusive der Anastomosentechnik. Während sich weltweit der J‑Pouch aus technischen Gründen durchgesetzt hat, herrscht bei der pouchanalen Anastomosentechnik (manuell oder maschinell) nach wie vor kein Konsens. Die mesorektumerhaltende Proktektomie und die maschinelle ileopouchanale Anastomose scheinen funktionell bessere Ergebnisse zu erzielen. Eine interdisziplinäre Betreuung der Patient*innen, insbesondere die Nachsorge des Pouches in einem Zentrum, ist unabdingbar.
