Ultraschall in der Diagnostik maligner Lymphome

Zusammenfassung

In der Diagnostik maligner Lymphome dient die Sonographie zur Detektion und Charakterisierung peripherer und abdomineller Lymphknoten sowie möglicher Organinfiltrationen. Hierzu werden neben B-Bild-Kriterien wie Größe, Form und Anzahl auch ihre Anordnung, Architektur, Echogenität sowie Abgrenzung zur Umgebung herangezogen. Mittels farbkodierter Dopplersonographie, B-Flow oder kontrastverstärkter Sonographie (CEUS) werden der Vaskularisationsgrad sowie die Gefäßarchitektur darstellbar. Wenn immer möglich, sollten hochfrequente Schallköpfe eingesetzt werden: Dies gilt auch in der abdominellen Organdiagnostik, v. a. bei der Detektion von Milzinfiltraten. Bei Organbeteiligung können Ultraschallkontrastmittel eine zusätzliche Hilfe sowohl bei der Detektion als auch der Charakterisierung lymphatischer Organinfiltrationen darstellen. Die Sonographie ist ein ideales Verfahren für die gezielte Punktion von Lymphomen, wobei Schneidbiopsien bevorzugt werden sollten. Die Kontrastmittelsonographie kann ferner zum Nachweis der Vitalität von Tumorgewebe unter oder nach erfolgter Chemotherapie eingesetzt werden.
