Operative Therapie am Becken und an der Wirbelsäule bei primär malignen Knochentumoren

Zusammenfassung

Trotz weitgehender Standardisierung der operativen Therapie von primär malignen Knochentumoren im Becken und an der Wirbelsäule ist die Prognose bei diesen zentral gelegenen Tumoren deutlich schlechter als bei Lokalisation im Bereich der Extremitäten. Dies ist einerseits auf eine in vielen Fällen verzögerte Diagnose mit entsprechend größerem Tumorvolumen, andererseits auf die Schwierigkeit der Resektion aufgrund der anatomischen Lage zurückzuführen.

Lokalrezidivraten bei Resektion von primär malignen Beckentumoren werden mit 4–41%, die 5-Jahres-Überlebensraten mit <40% angegeben. Während letztere in den vergangenen 20 Jahren weitgehend unverändert blieben, sind Amputationen bis auf einen geringen Teil von <10% durch extremitätenerhaltende Eingriffe ersetzt worden. Für die Rekonstruktion werden aufgrund der hohen Komplikationsrate bei Verwendung von Endoprothesen oder homologen Knochentransplantaten derzeit eher biologischen Rekonstruktionsverfahren bevorzugt.

Auch an der Wirbelsäule ist eine weite Resektion von Tumoren möglich, sofern zumindest ein Wirbelbogen tumorfrei ist, um die neuralen Strukturen aus dem Präparat befreien zu können, ohne den Tumor zu eröffnen. Im Vergleich zur weit verbreiteten intraläsionalen Curettage lässt sich mit der En-bloc-Resektion ein signifikant besseres onkologisches Ergebnis in bezug auf die lokale Tumorkontrolle erzielen. Die häufigste Komplikation dieser aufwendigen Eingriffe sind Implantatversagen bei verzögertem Durchbau der ventrodorsalen Rekonstruktion.

Zur Verbesserung der Langzeitprognose dieser Patientengruppen ist die Frühdiagnose und unverzügliche Zuweisung in ein Tumorzentrum anzustreben.
