Lokal-ablative Therapieverfahren neuroendokriner Lebermetastasen

Zusammenfassung

Hepatische Metastasen neuroendokriner Tumoren stellen einen negativen prognostischen Faktor sowohl für die Überlebenszeit als auch die Lebensqualität der Patienten dar. Die Rationale der lokalen Behandlung der hepatischen Filiae ergibt sich aus der häufig auf die Leber beschränkten Metastasierung oder durch medikamentös nicht beherrschbare Symptome der neuroendokrinen Lebermetastasen. Als Alternative zur chirurgischen Resektion der Metastasen wurden eine Reihe von lokalen Therapieverfahren entwickelt. Zu den etablierten Verfahren zählen die Embolisation und Chemoembolisation, die sich wegen der Hypervaskularisation und hepatisch-arteriellen Gefäßversorgung anbieten, und die perkutane Alkoholinjektion. Neuere Verfahren sind die Radiofrequenztherapie, die laserinduzierte interstitielle Tumortherapie und die Kryotherapie. Nachfolgend werden die einzelnen Verfahren, ihre Indikationen, Ergebnisse und Komplikationen hinsichtlich der Behandlung von Lebermetastasen neuroendokriner Tumoren vorgestellt.

Auch myeloide Immunzellen lassen sich mit chimären Antigenrezeptoren gegen Tumoren ausstatten. Solche CAR-Fresszell-Therapien werden jetzt für solide Tumoren entwickelt. Künftig soll dieser Prozess nicht mehr ex vivo, sondern per mRNA im Körper der Betroffenen erfolgen.

Blutdrucksenkung könnte Uterusmyome verhindern

Frauen mit unbehandelter oder neu auftretender Hypertonie haben ein deutlich erhöhtes Risiko für Uterusmyome. Eine Therapie mit Antihypertensiva geht hingegen mit einer verringerten Inzidenz der gutartigen Tumoren einher.
