Zusammenfassung

Defekte des Mittelzügels der Streckaponeurose können im Rahmen offener oder auch geschlossener Verletzungen, wie z. B. einer Fingerluxation im proximalen Interphalangeal(PIP)-Gelenk nach palmar, auftreten. Für eine optimale Versorgung ist die Identifikation aller verletzten Strukturen unabdingbar. Wird eine adäquate Primärtherapie versäumt, kann eine subakute bis chronische posttraumatische Knopflochdeformität entstehen, bei deren fixierter Form die Gelenkbeweglichkeit im PIP- und distalen Interphalangeal(DIP)-Gelenk zunächst wiederhergestellt werden muss. Im Anschluss kann eine sekundäre Rekonstruktion des Mittelzügels mit Sehnenplastik oder -transfer durchgeführt werden.
