Patellofemoraler Ersatz und Schlittenprothese

Zusammenfassung

Hintergrund

Etwa 20 % aller Patienten sind nach einem totalen Kniegelenkersatz mit dem Operationsergebnis unzufrieden.

Fragestellung

Stellt die individualisierte Versorgung eine Möglichkeit dar, die Gesamtzufriedenheit zu steigern?

Material und Methode

Als Alternative zum totalen Kniegelenkersatz führten wir eine doppelblind randomisierte Studie durch, die den totalen Kniegelenkersatz mit der Kombination „mediale Schlittenprothese und Patellofemoralersatz“ verglich. Je 40 Patienten mit medialer und patellofemoraler Gonarthrose wurden in die Studie aufgenommen. Präoperative Untersuchung, Indikationsstellung und Technik werden beschrieben.

Ergebnisse

Die klinischen Ergebnisse zeigten im postoperativen Verlauf eine signifikant bessere Kniegelenkbeweglichkeit der Kombination mediale Schlittenprothese und Patellofemoralersatz gegenüber dem totalen Kniegelenkersatz.

Schlussfolgerung

Unsere bisherigen Ergebnisse sind sehr vielversprechend und decken sich mit anderen Studien. Diese kurzfristigen Ergebnisse erfordern jedoch die Validierung in randomisierten Studien mit längeren Nachuntersuchungen.
