Primäre Degeneration des ZNS im Alter

Bildgebung charakteristischer Atrophiemuster

Zusammenfassung

Trotz der Weiterentwicklung innovativer magnetresonanztomographischer Methoden wie der Diffusionstensormessung und der 1H-Magnetresonanzspektroskopie hat die strukturelle Bildgebung nach wie vor einen zentralen Stellenwert in der Diagnostik altersassoziierter pathologischer Degenerationen des Zentralnervensystems. Es werden typische bildmorphologische Muster bei Multisystematrophien und primär demenziellen Erkrankungen aufgezeigt und differenzialdiagnostische Merkmale dargestellt. Diese haben insbesondere bei Querschnittuntersuchungen eine hohe Spezifität, jedoch geringe Sensitivität. Verlaufs- oder Längsschnittuntersuchungen können zwar einerseits die Dynamik und mitunter charakteristische Befunde besser darstellen, andererseits zeigt sich sowohl bei den Multisystematrophien mit initial betont motorischer Symptomatik als auch bei Demenzen mit zunehmender Krankheitsdauer eine Konvergenz und Überlappung der Atrophiemuster.
