The quest for the perfect hernia repair seems like a never-ending one. One would expect that evidence based surgery would by now lead us to a solution that is at least near-perfect. The introduction of techniques based on the use of synthetic mesh led us to believe, for a short while, that inguinal hernia repair is not a problem anymore. Laparoscopic repair than appeared and ignited the debate again. But one often tends to forget that what is commonly practiced in developed countries, represents only the tip of the iceberg. Availability and cost considerations, which are often neglected in scientific literature, pose a major obstacle to the adoption of “modern” surgical techniques in less privileged countries. …
