Rehabilitation nach distaler Radiusfraktur

Zusammenfassung

Hintergrund

Distale Radiusfrakturen zählen zu den häufigsten Frakturen des Erwachsenen. Die Behandlung erfolgt je nach Frakturtyp, Alter und Aktivität des Patienten konservativ oder operativ. Anschließend erfolgt die Rehabilitation in Form von Übungen zur Wiederherstellung des Bewegungsumfangs, der Kraft und der Geschicklichkeit in den Verrichtungen des alltäglichen Lebens. Erfolgt diese nicht, sind Bewegungseinschränkungen und Kraftminderungen keine Seltenheit. Im klinischen Alltag erfolgt die routinemäßige Verschreibung von Physiotherapie. Die aktuelle Datenlage bietet keine ausreichende Evidenz, inwiefern diese erfolgen sollte.

Ziel

Ziel des Beitrags ist eine Darstellung der aktuellen Studienlage zur Nachbehandlung nach konservativ sowie operativ versorgter distaler Radiusfraktur, ergänzt durch das Beispiel des eigenen Vorgehens.

Ergebnisse

Die vorliegenden Studien weisen darauf hin, dass eine physiotherapeutische Nachbehandlung nach einfachen distalen Radiusfrakturen, konservativ oder operativ versorgt, mit einem besseren Outcome einher geht als Frakturen ohne Nachbehandlung. Die eigenständige Behandlung nach einem vorgegebenen Schema geht dabei mit gleichen Resultaten einher wie eine durch den Physiotherapeuten durchgeführte Behandlung. Komplexe Frakturen zeigen ein besseres Outcome unter Physiotherapie. Ergänzt durch regelmäßige, häusliche, eigenständige Beübung wird das Outcome der Patienten weiterhin gesteigert.

Schlussfolgerung

Eine gezielte Nachbehandlung nach distalen Radiusfrakturen verbessert das Outcome der Patienten bezüglich Bewegungsumfang, Kraft und Greiffunktion. Diese ist bei einfachen Frakturen in eigenständiger Durchführung ausreichend, bei komplexen Frakturen durch Physiotherapeuten zu empfehlen.
