Rekonstruktion des medialen Bandapparats am Ellenbogen bei posttraumatischen und chronischen Bandinstabilitäten

Zusammenfassung

Operationsziel

Wiederherstellung der ulnaren Ellenbogenstabilität bei posttraumatischer und chronisch-rezidivierender medialer Bandinstabilität.

Indikationen

Symptomatische therapieresistente mediale Bandinstabilität des Ellenbogens.

Kontraindikationen

Arthrofibrose und höhergradige Arthrose.

Operationstechnik

Anatomische Rekonstruktion des medialen Bandapparats (MCL) mit einem transossär verankerten autologen Sehnentransplantat (Sehnen des M. palmaris longus und M. gracilis) in implantatfreier Technik.

Nachbehandlung

Oberarm-Cast für eine Woche, 5 Wochen Orthese mit Bewegungslimitation, Vollbelastung nach 12 Wochen.

Ergebnisse

Insgesamt 90 % der Patienten erreichen nach 2 Jahren wieder das alte Aktivitätsniveau. Die Gesamtkomplikationsrate liegt bei etwa 10,4 %. Hauptproblem ist mit ca. 7,8 % die N.-ulnaris-Affektion.
