Septische Enzephalopathie

Zusammenfassung

Die septischen Enzephalopathie ist die häufigste Enzephalopathie auf Intensivstationen und bezeichnet eine diffuse zerebrale Funktionsstörung im Rahmen einer Sepsis nach Ausschluss anderer Ursachen. Leitsymptom ist die Bewusstseinsstörung, epileptische Anfälle können auftreten, herdneurologische Störungen sind selten. Die Pathogenese ist multifaktoriell und nicht vollständig geklärt, wobei zerebrale Perfusionsstörungen eine wichtige Rolle spielen. Neuropathologisch dominieren Ischämie und Inflammation in verschiedenen Arealen ohne Erregernachweis. Zusatzuntersuchungen dienen dem Ausschluss konkurrierender Ursachen, insbesondere entzündlicher ZNS-Erkrankungen. Das zerebrale CT ist unauffällig, im MRT des Schädels konnte z. T. ein Hirnödem mit posteriorer Akzentuierung im Sinne eines posterioren reversiblen Enzephelopathiesyndroms nachgewiesen werden. Die Therapie der septischen Enzephalopathie entspricht der Therapie der Sepsis, spezifischere Therapieansätze sind Gegenstand aktueller Untersuchungen.
