Surgical dogma has historically mandated operative management of small bowel obstruction (SBO) in patients with no previous history of abdominal operations, due to potential malignancy as the underlying etiology [
1]. However, recent data suggest that adhesive disease is the leading cause of SBO in these patients, similar to those with a surgical history [
