Technik der Proktokolektomie mit ileoanalem Pouch

Zusammenfassung

Hintergrund

Die restaurative Proktokolektomie (RPC) gilt als Standardoperation im Falle der therapierefraktären Situation und bei Neoplasie bei Colitis ulcerosa (CU).

Fragestellung

Ziel der Arbeit war es, anhand der aktuellen Evidenz einen Überblick über Standards, Innovationen und Kontroversen in Bezug auf die Technik der RPC zu geben.

Ergebnisse

Die RPC mit ileoanaler Pouchanlage (IAP) stellt die chirurgische Standardtherapie bei therapierefraktärer CU und bei Neoplasie dar. Wegen der chirurgisch einfachen Anlagetechnik und guten funktionellen Ergebnissen hat sich der J‑Pouch als Reservoir durchgesetzt. In der Regel erfolgt die RPC zweizeitig, bei Risikopatienten dreizeitig. Die technisch aufwendigere komplette Mukosektomie mit Handnaht ist der Stapleranastomose onkologisch nicht eindeutig überlegen. Nach Stapleranastomose scheinen die funktionellen Ergebnisse tendenziell günstiger zu sein, die Restrektumlänge sollte hier aber 2 cm nicht überschreiten.
Der laparoskopische Zugang ist dem offenen mindestens gleichwertig. Für neue, innovative, Operationstechniken wie robotische Chirurgie und transanale Verfahren wurden bisher nur die Machbarkeit, aber keine Vorteile gezeigt.

Schlussfolgerungen

Die Evidenz zu kontrovers diskutierten Fragen bei der RPC ist begrenzt.
