The utopia of randomized controlled trials: tissue repair of umbilical hernias is still feasible

In a recent meta-analysis of randomized controlled trials (RCTs) comparing open
vs. suture repair of umbilical hernias (UHs), Aiolfi et al. systematically analyzed six RCTs strongly in favor of mesh repair. This analysis included 742 total patients; (mesh = 383); (tissue repair = 359). The overall rate of recurrence was 6.3%. The estimated pooled postoperative RR was 0.27 (95% CI 0.13 to 0.53;
p < 0.001). The estimated relative risk for complications did not reach statistical significance [
1]. Close examination of all RCTs of tissue vs. mesh repair for umbilical hernia repairs (UHRs) demonstrates a recurrence rate of (0.0% to 4.0%) with mesh vs
. (7.0% to 17.0%) with tissue repair. These studies had a wide variability in the use of mesh repair as well as the technique used for UHR. Only the most recent RCT had a training session for all participant surgeons in an attempt to standardize the techniques employed [
